Check the VGA cable on both the monitor and KVS.
The VGA cable is a blue display cable and should be connected to both the monitor and an adapter on the KVS. Reseat the cable in both ports and reboot the controller. NOTE: on KVS controller VGA adapter is fragile-- be careful!
